The form should be designed in such a manner that it facilitates in gathering all the information needed regarding the condition of a patient. For the forms, they help the medical staff to assess the information regarding the health situation of a patient conveniently. For the paper, it critiques an Intake Mental Health Form through answering questions related to it.

For mental health forms, they need to make sure that they incorporate all the needed information pertaining to the health condition by a patient. Asking too many or too few questions in a mental health form might be problematic.
