Relevant factors should be identified, both internal and external, that might have caused the fire outbreak. After the elements have been identified, a list of options is developed so that it can help to avoid the dilemma of what to decide and say. Therefore, if the dilemma is avoided, it will be easy to identify the cause of the fire outbreak by testing all the options thought to have been the cause of the fire outbreak. Whoever was behind the fire outbreak made sure all the communications were lost, and the fire alarms were all cut off so that the response to the fire outbreak would be slow and that people would have the problem of reaching to each other to enquire what was happening. I recommend that all the employees be interviewed and identify the relationship between all the employees and the management team to identify the cause of the problem. The interviews will help determine if the relationship between the management and the employees was healthy or unhealthy.
