Why is quality difficult to define? Because it is defined as both measure of input as well as the results or outcome. From the 1960s, evaluation of quality has been assessed in three aspects: process of long-term care, structure and the outcomes of care. While the quality of nursing homes are being measured by structural variables like the mix and level of education as well as training of the employees (Pratt, 2016, p. 5) . 

Why is quality of life of particular importance in long term care? Because one aspect of defining the quality of long-term care is assessing the outcome of the particular care and successful outcomes are often based on factors like maximizing the quality of life as well as the proper physical functioning (Pratt, 2016, p. 5) .

What are the purpose and functions of a quality assurance committee? Quality assurance and assessment committee represent the major points of responsibility and accountability in ascertaining both the quality of care as well as life in long-term nursing care (CMS, 2016) . 

How does process, structure and outcomes measures differ? The three measures of assessing quality of care; process, structure and outcome measures differ in several ways. Process measure evaluates the care services offered to the customer, outcome measure comprises of changes in health condition and status due to the care offered to the resident (Pratt, 2016, p. 19) . Structure measures include mix, education and training of employees, level and then characteristics of the facilities. 

What are the differences between QA and CQI? Quality assurance is the process that examines functions with the objective of identifying and rectifying divergence from the stipulated policy. While continuous quality improvement is an approach developed based on partnership between individuals engaged in application of the skills and the one conducting the CQI process (Pratt, 2016, p. 371) . The similarity between the two is that both are mutually seeking to improve the facility and its staff in achieving the desired outcomes. 

What are quality indicators? Quality indicators use elements of data to develop measures that can be transformed into statistical summaries. 

What are incentives, if any for long term providers in terms of overall system? Incentives with care providers include shortening Medicaid staying when capacity shortages arises. 

What disincentives exist in the payment system when it comes to improving quality? Disincentives that exist in the payment system include pay-for-performance which applies mostly to primary care physicians (CMS, 2016) . It can be overcome by calculating the rewards as continuous function of the total number of residents who received long-term care.
