Quality Care was not met according to one of the surveys carried out in the facility. The survey identified that the facility failed to adhere to the required professional standards of patient care in two of six patients exanimated for quality care experiences. In the first patient, professional standards did not apply as he was not accorded with timely care due to a change of condition during their stay in the healthcare facility. On the other hand, the second patient had also experienced an emergency decline in the healthcare providers' condition to address promptly. In general, both patients were not provided a timely response for their emergencies, resulting in a violation of the professional standards of quality care.
Question 2. If you were the manager on the unit where the most egregious deficiency occurred, what would you have done differently to correct/prevent the issue from occurring again. This must be different than what the plan of correction already states.
As the manager of Pontiac Nursing Home healthcare facility, the above-described incidence of eluding professional standards of quality care delivery would have been avoided using the following plan of action;
First, patients' change in condition is a huge concern for healthcare professionals and should be treated immediately due to its severe consequences, including fatality. The inspection report carried out on Pontiac Nursing Home indicated that the healthcare providers failed to adhere to professional standards of quality care, thus creating room for healthcare setting incompetence. However, the care providers should have ensured clear communication channels between nurses changing shifts and creating good records that show every change concerning a specific patient ( Messinger-Rapport, 2004 ). Effective communication includes being in touch with the patients in the healthcare facility to ensure good rapport and for residents to feel more secure in the care facility, unlike their homes. I would ensure that care services offered to patients are directly related to their current conditions; this prompts a quicker response from medical professionals during emergency incidents that require rapid response.
Secondly, it is my responsibility as the manager to ensure the smooth flow of care delivery to patients and cross-check on what sectors need improvement. Consequently, my primary focus would be providing the healthcare providers with a good and safe working environment to ensure they are motivated to serve residents in the right and timely manner, especially during emergencies like an immediate change of conditions ( Boscart et al. 2018 ). Conducting a medical record review for patients is another strategy that I would use to ensure no discrepancies are hidden within the facility's health provision records.

After going through the surveyor’s report, I have some significant concerns regarding the surveyor's writing of deficient practices. The side of deficiencies cited is well-written and represented the surveyor’s fair opinion on the facilities practices and deficiencies. The deficiencies cited are explained with relevance to specific surveys, thus shows the surveyors fairness in identifying available discrepancies. Nevertheless, the surveyor is not fair enough when making relevant suggestions on defiant practices that include action plans respective for each deficiency identified. As seen in the report, some crucial deficiencies were not provided with action plans, thus rendering the surveyors' actions on the provision of action plan unfair. The surveyor should have covered every deficiency with a respective plan to ensure fairness; this would have also promoted the report's usefulness to the Pontiac Nursing Home.

This report provides a lot of informative information regarding nursing practices and professional standards of quality care. After reading the report, I have developed an in-depth understanding of essential prerequisites and services offered at Nursing Home facilities, the professional standards expected from nurse practitioners working at the facility, and the importance of keeping healthcare operations in recorded formats nurse shifts and duty allocation. The report provided useful experience for my nursing practice in the following ways;
I managed to understand the essentiality of emergency services and understanding requirements as per the professional care standards. On the emergency of services, I am more concerned about taking emergency services seriously and promptly ensuring the emergency necessities. On the other hand, as a nurse practitioner, professional standards are vital in promoting efficient care delivery and ensuring nursing home residents enjoy maximum comfort.
In summary, the report is an informative text that explains various procedures and processes that influence care delivery in nursing homes. Significant deficiencies that primarily exist in nursing homes are explicitly covered and defined; some matched the individual plan of action that nursing homes can embrace for the success of operations and care delivery processes.
