The identified model of nursing care is the Synergy Model for Patient Care. From the observation witnessed from the model is that the model facilitates in connecting the traits of the patients with the expertise of the nurses in an effort to realize optimal outcomes from the patients. The model is mostly applicable for patients in critical and acute care environments. The reason behind choosing the model revolves around the idea where it provides nurses with an opportunity for delivering care beyond hospital environments, particularly to patients at home, hence making it possible to maintain life quality in line with minimizing the costs related to hospital readmissions.

For the model, the observations made relate to staff educating patients on insulin injection while using a sliding scale. The major observation relates to the situation where monitoring of blood glucose usually occurs at set times, such as during bed times, prior to means, or at certain times at time. It is essential to hold discussions with patients, assess the outpatient records of the patient, and ensure to discuss with the available care providers of the patient to offer a starting point for evaluating self-management’s appropriateness. It is also crucial to assess the knowledge of the patent to determine their competence in managing diabetes on their own. In the case of the patients who show deficits in knowledge or are incapable of managing their diabetes because of a prevailing disease or side effects from medication, they should not be permitted to manage their diabetes condition while in the healthcare setting. Furthermore, it is crucial to realize that hospitalizing these kinds of patients plats an essential role in addressing the knowledge gaps that prevail among the patients. 

When it comes to the patients having the physical capacity and knowledge on diabetes self-management through insulin injection, they should be provided with the opportunity for self-managing their condition when probable. This is particularly the case for the patients with Type 1 and Type 2 diabetes who have basal bolus insulin under adequate control for outpatient individuals. By providing the patients with a means of self-managing the diabetic condition through insulin injection, they get the best capability for coordinating blood glucose, nutrient consumption, and administration of insulin, which allows them to realize improved outcomes. 

In making sure that the self-management endeavor by the patient emerges safe and effective, it is crucial to make sure a hospital policy prevails while making sure that the patient supports the idea of follow the policy. It also becomes important to document the things that patients are permitted to do and what they actually do to facilitate in keeping clear records while at the same time determining the kind of care required when offering care. Since the policies that need the utilization of sliding scales and requirement for the nurses in storing insulin might serve as barriers to coordinating car among outpatients, it is vital to have guidelines. These should detail the blood glucose test numbers that utilize the sliding scales and nursing documentation of insulin administration to patients. These should feature defining policies that feature patient agreements in order to create room for flexibility when caring for the patients.

The scholarly resource associated with the nursing care model observed is “Adaptation of the AACN Synergy Model for Patient Care to Critical Care Transport.” According to the article, the system of delivering healthcare in the present world mostly relies on inter-hospital transfer of individuals needing increased care levels. It thus advocates on the need for having a transfer system that would facilitate in transferring patients between healthcare institutions efficient to make sure that they receive effective and safe care (Swickard, Swickard, Reimer, Lindell, & Winkelman, 2014) . For the model, it relates to the observed nursing care model owing to how it requires healthcare institutions should have a system of transferring patients between facilities. Here, when educating patients concerning insulin injection to allow them to manage their diabetic condition adequately, this serves as a means of ensuring patients realize effective and safe care, which is the major area of focus by various healthcare organizations.

A resource related to a distinct nursing model observed in the practice setting is “Patient Navigation as a Model to Increase Participation of African Americans in Cancer Clinical Trials.” The source stipulates that the Patient Navigation Model has played an essential role in boosting medical care access although no evaluations have prevailed on improving minority participation during clinical trials (Fouad, et al., 2016) . The model aims at promising an approach that would ensure that disparities reduce during trial participations among cancer patients. When it comes to this article, it applies in the area of educating patients on insulin injection to allow them in managing their condition. For the model, it requires that patients should have the capacity for accessing medical care in line with making sure that disparities reduce when accessing care. In this case, ensuring that patients have knowledge on insulin injection makes it possible to self-manage their condition without requiring doctor’s services regularly. In this manner, they would be able to cater for their situation when they need in line with addressing their condition when a need arises. In this manner, disparities minimize in accessing insulin injection services thus boosting chances of accessing healthcare.

From step #6, the nursing care model that differs from the one observed in the practice is the Case Management Model. The model emphasizes on the need for showing emphasis on caring for the patients as opposed to relationship that exists between nurses and patients. The model can boost quality since the nurses would specialize on making sure that patients receive quality care. It would also boost safety and participation when patients recover and manage to perform their regular duties (Elder, Jacobs, & Fast, 2015) . In the event of the Case Management Model, it is crucial to delivery of care while it is significantly essential among the patients receiving education on insulin injection to allow them to self-manage their condition. In the event of the model, it requires that major emphasis be directed to patient care while educating patients about insulin injection serves as a major area of emphasis when it comes to making sure that patients receive adequate, quality, and safe care. 

Summarize this assignment and what you learned about the two nursing care models discussed. 

From the two nursing models, the thing learned is that it is crucial to focus on delivering care to patients, particularly those with severe illnesses, by offering them avenues through which they can access care easily, including homes or through the provision of reliable hospital transfer services. This ensures that patients receive quality, effective, and safe care. In the event of the two nursing models, I have realized that they contribute greatly to self-management among patients through insulin injection. With knowledge of insulin injection the patients would be able to cater for their condition anywhere thereby making sure that they remain productive in the diverse activities that they undertake without experiencing hefty medical costs
