The healthcare system in the United States has, for many years, been faced with intractable problems (Clarke, Bourn, Skoufalos, Beck & Castillo, 2017). These problems include bias and uneven access to healthcare, payment of unrewarding incentives, and a less coordinated healthcare system, which has attributed to several safety risks and a notable disconnect between quality and cost. The consequence of this has been the increase in the price of healthcare, the limitation of productivity and efficiency, and customer dissatisfaction. The overreaching effect of this has been the degeneration of the value within the health care system triggering various reform proposals, with many of them rooted in regulating the cost of healthcare and providing quick access to care. One of the new models, the Patient-Centered Medical Home healthcare mechanisms, has facilitated the increase in the value of care delivery.

Patient-Centered Medical Home (PCMH) is an innovative patient-centered model that is specially designed to provide valuable care to patients by strictly optimizing healthcare for each individual. PCMH model integrates the Interdisciplinary care team (ICT), comprising of physicians, nurses, pharmacists, nutritionists and dietitians, behaviors, and mental health specialists as well as community support. In this new model care design, personalized care plans work to determine future schedules, reviewing safety measures, and analyzing clinical records via the Electronic Health Records. PCMH supports health information technology innovations paving the way for a functional and round the clock specialized care services to consumers. This model comprises of funded nursing care, professional analytics to identify risk trends and provide proactive, evidence-based care to minimize the cost of hospitalization (Clarke et al., 2017). This new clinical model design is advantageous to patients in that it provides an efficient decision support system that actively engages patients and closely tracks their medical performance, thus putting the care quality in check.
