Procedure for obtaining the health records (PHR) 

One is required to serve a subpoena to the healthcare provider’s office to get their medical records. Often, this can be through a lawyer, who is required by law to prove that the client has instructed them, and must clarify the indented use of the records (Fragidis & Chatzoglou, 2018). According to Section 456.057 of Florida Statutes, the healthcare provider is expected to channel the documents to the patient reasonably without delay (Price et al., 2019). The law dictates maximum confidentiality, as the records can only be issued to any other party under the client’s acknowledgment, federal and state law agencies, or other health facilities in need of the data when attending to the patient. 

The Cost  

According to the Health Insurance Portability and Accountability Act (HIPAA), the charges of accessing personal health records should not exceed USD1 for the first 25 pages. Besides, subsequent pages should cost no more than USD0.25 per page (Pang et al., 2020). HIPAA also requires that the total must not exceed the cost of copying. A fee of USD1 per year could also be charged for safekeeping.

Choice of the type and format of the PHR 

I will use the standalone type of Personal Health Records, which allows me to have my records on my device. I can then update them and use third-party services like Google Health to make them shareable with health care organizations of choice. I chose the standalone type because it is more customizable, adaptable, and customizable since I can add modules like diet plans (Woollen et al., 2016). the standalone PHR can also accept data from external sources like laboratories. Besides, I prefer the digital format since it allows me to use smart technologies like block chain and artificial intelligence (AI) from Google Health to obtain meaningful suggestions on improving my wellbeing.
