Pressure ulcers are injuries that affect the skin and other underlying tissues. The small injuries affect the tissues by breaking them down and they occur when one or different areas of the skin are put under pressure. They can occur when a lot of pressure is applied on the skin for a short time or when less amount of pressure is applied on skin for a long time. It injuries occur because the pressure acts as a disruption to the flow of blood through the skin. The severity of pressure ulcers ranges from small colored patches on the skin to big open wounds that expose the underlying skin tissues.
The need identification for the project was through combined method because I used every all the available resources including study material and other students to come up with the idea for the project. This also involved identifying the possible problems associated with the topic of study in order to cut on both the cost and time of research. The main significance of need identification is to observe and gather information about the topic of study. Observation helps one answer the most critical questions about the project while gathering information helps one understand and highlight all the processes and procedures required in order to successfully execute the proposed project.

The most effective method to measure the stages and outcomes of a project is to set specific, measurable and realistic goals. For this project the specific goal was to find several preventive measures for the disorder because it has been a nursing concern for many years. Secondly, the measurable goal for the project was to find out the number of young people who acquire pressure ulcers each year and compare it to the number of elderly people who experience it every year. This goal is important for prediction purposes. The realistic goal set for this project was to treat at least 40% of patients suffering from the disorder, using the available resources and personnel each year.
