What safety concerns exist for Max? 

Older patients like Max are at a higher risk of developing particular problems/complications immediately after surgery. They have a high risk of developing a heart attack, blood clots, getting confused, and in severe cases, death. Besides, there is a risk of developing an infection after undergoing hip replacement surgery. 

What interventions would we take to ensure Max's safety? 

To ensure Max's safety, any medical conditions that he might have should be managed before he undergoes hip replacement to reduce risks. Post-operation, Max should be closely monitored and assessed to identify any worsening in condition. If complications are not noticed, and no immediate intervention or treatment is provided, the patient might be harmed severely. They can harm the patient. Also, an appropriate postoperative nursing care plan should be executed. Max’s care plan should focus on preventing displacement of hip prosthesis, development of thrombosis, uncontrolled drainage of the wound, and infection.

Is Demerol a good choice for 70-year-old Max? Why or why not? 

Meperidine, sold as Demerol, is not the right choice for Max. Older adults are most likely to develop more severe effects associated with the medication. Firstly, older patients are likely to experience CNS side effects of the drug, such as confusion and anxiety, among others. Secondly, older patients have reduced renal function and are most likely to build up normeperidine. Normeperidine is removed through the renal system, and repeated dosing puts older patients at a high risk of toxicity. Meperidine is listed among the medications that older patients should avoid because it is correlated to unplanned hospitalizations. 

You are in a discharge meeting for Max, which is also attended by Max's wife, the RN leading the team today, a physical therapist, and a pharmacist. Max's wife would like him to return home but isn't sure how she will manage. What type of services could help Max safely return home? 

The transition from a hospital setting to home services would be essential for Max. Besides, visiting nurse services and home visits by other healthcare team members such as the physical therapists would be crucial. Also, the patient would require to be monitored and assessed regularly to prevent any complications. Other services, such as transport, would be required to help Max safely return home.
