The Let’s Get Healthy California goal ensures that everyone in the state has an opportunity to achieve better health. Another goal's aim is ensuring better equitable care across Californians' lifespan and pathways to health-from their children to their seniors. One of the important goals of Let’s Get Healthy California is redesigning the health system. The goal aims at ensuring that all Californians have access to efficient, safe, and patient-centered care through several indicators, such as increasing access to primary care providers (Butler, Dehner, Gates, Shane, Chu, DeMartini & Chen , 2017). 

Unfortunately, not all Californians have the opportunity to access primary care when in need (Butler et al., 2017). Data from the California Health and Human Services portal shows that the demand for health care is increasing. Therefore, the state needs additional primary care providers to meet patients' requirements. About seven million Californians live in areas with a rampant shortage of primary care providers (Butler et al., 2017). People in the affected areas are also unable to meet dental or mental healthcare providers timely. Even though insurance has, to some extent, provide access to care, it fails to ensure that every citizen in California receives appropriate care at the right time. The shortage of primary care professionals has negatively affected health outcomes due to frequent delays in obtaining timely health services. As a result, the physical, social, mental health, and overall quality of life has continued to deteriorate in the state of California (Mishra et al., 2016).

To some extent, the available health programs in Los Angeles strives to ensure they align with the goals of Let’s Get Healthy California. For instance, the CalFresh program has also enabled households to live well by offering a means to meet their nutritional needs. As a result, the overall incidences of obesity have reduced, and the overall health status of Californians has increased. Besides, the efforts of Community Health Programs and Partners in Los Angeles have ensured that Let’s Get Healthy California goals are successful. These goals include redesigning the health system, maintaining healthy communities, and reducing the cost of care. The Los Angeles Care’s Health Homes program also helps the patient meet some of their end of life goals. 

https://www.lacare.org/providers/provider-resources/health-homes-program . The Healthy Homes program provides team-based in-person care management to those that are chronically ill. The program members conduct advance care planning by promoting social services and behavioral health to meet the needs of the high-acuity medical population in the community.

The Community Health Care Programs and Partners were established in 1969 to provide support and health expertise that would help to improve the health of others. The programs promote health equity, chronic disease prevention and optimal aging, family health, and support patients with mental health needs.

CalFresh is a federal assistance program in Los Angeles that helps individuals and families experiencing food insecurity to gain access to nutritious food. The program has helped many low-income families in Los Angeles to put healthy food on their tables each month. Adults, adolescents, and children live well by eating fresh fruits and vegetables the program supplies.
