Data collection and data analysis are important in measuring the chronology of events when attending to patients in a health facility. When a patient visits a health facility, the responsible staff should collect the data relating to the personal details including names, age, date of birth, address, residential, illness and prescribed medication, and the unique identification number. The data collected should be analyzed and stored in an effective and efficient system in the organization. Analysis of the data will enable the nurses, doctors, and other workers in the health facility to identify the patient and ensure that the patient has a unique feature among the data collected ( Ross, 2013 ). Additionally, the analysis will enable the staff in the health facility to ensure that the patients admitted do not share the details but have at least one unique feature for identification. The data collection and analysis would enable the staff to identify the patient chronologically without any errors experienced. For example, two patients would have been identified using unique identification numbers, which are not shared by patients something that did not happen as only names were used in identification. 

The first step is ensuring that the health facility has an effective and efficient system that helps in storing and retrieving information about patients as well as the previous treatments. The next step has qualified staff that understands the procedure followed when collecting and analyzing data from the patient in the process of admitting the patient ( Ross, 2013 ). The next step is assigning a unique identification number to each patient in the process of admission, which will be used in the identification of the patient. The unique number will enable the staff in the health facility to access all the information about the patient including prescribed treatments when the number is keyed in the system.

Humans working in the health facility produce errors due to lack of communication when attending to the patients. In this case study, there was lack of communication between nurses, doctors, and physicians while attending to the patients leading to misidentification. Another challenge was reluctance among the staff, as they could not seek more information to identify the patient even after realizing the first error ( Ross, 2013 ). Health facility should establish effective and efficient communication system between the staff while attending to the patients. Additionally, the patients should not be transported to another unit within the health facility without an official order from the responsible staff. The staff should use unique identification numbers when seeking information about the patients.
