Sustainability refers to the continuation of programs by maintaining its effectiveness and impact over a period of time. Ensuring that a health program is sustainable is critical for its overall success. The health issue selected is obesity and the health program that has been created involves measuring the Body Mass Index (BMI) of a given population and providing advice on how different individuals can improve their weight. After the creation of the program, sustainability will be achieved through having a continued commitment by the participants to achieve the program's mission and goals, adequate resource management, and management of health team (Shelton et al., 2018). Adequate resources will be budgeted for and provided in order to ensure that the program continues for the long-term. The personnel and human resources required for the success of the program will be provided with the critical knowledge and attitudes. Collaboration among the health team will be considered of critical importance and the entire team will be encouraged to work together.

There are several creative ways to communicate a program to the public. The chosen approach will involve a multidimensional target-focused approach through various media channels and encouraging a target-focused approach. The multidimensional approach will involve the use of various media channels such as television, social media tools, the internet, radio, and newspapers. The use of multidimensional communication channels will also be important for a mass-reach of the health intervention. The communication will be target-focused by ensuring that it appeals to the intended audience. The target population will be used to dictate the communication channel to be used. For instance, the use of social media will target the young population that is active on the internet while newspapers will be mostly used for the elderly population. The specific message used for the communication will also be creatively created so that it can be relevant and appealing for the specific audience.
