A person dependent approach to promoting safe and effective nursing care that I have experienced in my practice is allowing patients to actively participate in their care. The approach involves patients and their families in the decision-making process to plan care and treatment. The inclusion of patients in decision-making, healthcare planning, and goal setting improves their healthcare. The healthcare professionals in my practice provide patients with all the information needed to make informed decisions as well as time and opportunities to ask questions, and time to talk to family, friends, and carers before making decisions. Active involvement of patients in care helps them and the healthcare team to plan and prioritize treatment in situations where many treatments are needed at the same time. Patients are also provided with the right to choose treatment options they are comfortable with, except when they cannot give their consent. Patients are given a right to appoint a person to make medical decisions for them when they lose the capacity to make decisions. 

The person dependent approach of allowing patients to actively participate in their care can be transformed from person dependent to system or process dependent by the building of teams to take the organization from the current state of care delivery to the ideal. The transformation depends not on individual providers working to fix a broken system, but a collective action of providers working together while breaking down silos ( Leyshon & McAdam, 2015) . The results of using person dependent approach are outcome improvement by actively engaging patients and families. As a result, providers who see the experience of family's and patient's points of view co-designs care delivery.

The advantage of transforming the approach to system or process dependent is to promote better health at lower costs. A system approach improves health by considering a wide range of elements necessary for quality care and the various factors influencing health (Kaplan et al. 2016). The system approach helps in designing and integrating processes, people, policies, and organizations to promote quality health at reduced costs by understanding the operation of healthcare elements both dependently and independently.
