Incidences of rehospitalization among geriatric patients in home health care has resulted in the need for telehealth to improve access to healthcare. Telehealth care is a developing sustainable intervention model in treating complex chronic conditions like congestive heart failure (CHF) and chronic obstructive pulmonary disease (COPD), and to involve elderly patients in self-care disease management ( Hale, Jethwani & Kandola, et al., 2016). 

Rehospitalization leads to huge medical costs expenses that are channeled towards catering for these patients thus the need for telehealth homecare. COPD and CHF need complex treatment procedures that are very costly. Home health care patients includes a group of homebound older adults representing a high medical cost, medically fail, isolated, and diverse group. This group of people often have limited access to care, many medical conditions, and financial stressors ( Gellis & Kenaley, 2017).

Telehealth monitoring improves access of care for patient in homebound settings. Various studies have revealed that the use of telehealth monitoring in home health has shown significant improvement in social functioning and general health of geriatric patients. Evidence reveals that patients who have used telehealth have been found to spend fewer days in hospitals compared to those who do not use it. Generally, the maximum number of days for home healthcare is 90 days of care for every single patient. This means that the use of telehealth care technology in home healthcare and release of patients before the 90 days period elapses has a high chance of improving the general health of patients. In addition, telehealth enhances patient engagement and satisfaction. More convenient care makes patients satisfied and happy hence become more engaged to their care. Therefore, T elehealth is an effective and efficient method for delivering quality care to older adults in home health sector. Specifically, this is beneficial to older adults who are homebound and experience challenges accessing care because of isolation, transportation, and disability ( Gellis & Kenaley, 2017).
