End Shift Report 

The end shift report concerning Tina assessement include history of heart disease in Tina’s family. The boold for Tina was elevate, the scrape was swollen, and had other conditions including diabetes and asthma. Tina is also experiencing difficults in maintain clear eye site when reading. She had not been having a balanced diate and does not conduct physical activities. 

SMART Goals 

The two SMART goals for Tina include immediate action and reducing risk factors. The immediate action goal will involve conducting medical interventions to reduce the conditions that might cause further damage. The first goal will be followed by lowering risk factors goal, which involves controlling risk factors such as high blood pressure, diabetes, obesity, and lack of physical activity regularly (Black, 2018). The aspects included in the risk factors reduction goal should be specific, with time and duration, measurable within a period, can be attainable based on the state of the patient, and be realistic.

Evidence-Based Nursing Interventions 

The evidence-based nursing interventions that can be applied include cognitive-behavioral intervention, which provides for educational and coping skills training. The cognitive-behavioral intervention incorporates providing support for physical activity and other behaviors such as eating. Follow up through a telephone call accompanies the intervention sessions (Chyun, 2014). Another evidence-based nursing intervention is research, which involves the nurse studying different gaps in healthcare and explores them to generate knowledge that can be used to enhance patient health.
