Lack of engagement in exercises and falls among the elderly population in California have been noted as key contributors to serious health complications and risks, some of which lead to an increase in the mortality rates significantly. That has created the need for having to focus on an action plan that would be of value towards educating the community on exercise and fall prevention in the elderly. Development of the action plan focused on community education will be considered as a steadfast approach through which to ensure that effective nursing interventions are put in place to increase engagement in exercise and prevent falls. The project will be undertaken at the community level with the sole aim being towards creating an advanced platform through which different persons in the community are involved in the adoption and effective utilization of the nursing interventions suggested.

The first goal of the project revolves around the provision of knowledge among members of the community on the value placed on exercise and fall prevention with the focus being on the elderly population. Through education, members of the community will be in a better position through which to get a clear understanding of the health risks that the elderly population is exposed to from lack of exercise and an increasing number of falls.

Enhance the adoption of effective nursing actions/interventions to help to deal with the health problem indicated 

The crucial second goal of the problem is to increase acceptance of nursing action/interventions that would be of value towards increasing engagement in exercises and reducing exposure to falls among the elderly persons in the community. The nursing interventions will play a critical role in ensuring that indeed, the elderly population is well protected from the health risks that they face due to lack of exercise and an increase in cases of falls.

Implementation Plan 

The implementation plan for the community education project will focus solely on the idea of engaging with members of the community as a way of ensuring that they attend the training and development programs. That means that it will be essential to come up with an approach through which to expose the health risks facing the elderly population resulting from lack of exercise and falls. The expectation is that this will help create some form of understanding among the community members, which will help push them towards getting involved in the project.

What is important to note is that a majority of the elderly persons in the community have been affected by health complications associated with exercise and falls. Consequently, this builds on the understanding that indeed, a majority of the people within California may find themselves experiencing the need for education on exercise and fall prevention in the elderly. Establishing a front for community engagement would be of great value in highlighting the importance of knowledge on the topic at hand. Additionally, community engagement will also help in ensuring that the activities that will be put in place as part of the educational project resonate with members of the community. In other words, the activities undertaken as part of the project must have a close connection to the expectations not only for the elderly population but also for other persons within the community setting.

Justification of the Project 

Lack of knowledge and information touching on exercise and fall prevention acts as a risk factor contributing to the problem in question. Ungar, Rafanelli, Iacomelli, Brunetti, Ceccofiglio, Tesi, & Marchionni (2013) take note of the fact that one of the key factors contributing to the increasing number of falls among elderly persons is lack of knowledge on how the falls would be prevented. From this perspective, what is clear is that a majority of the elderly persons in the community are suffering due to lack of information on how the issues could be averted. Undertaking this project will be of great value for the community, as it will help provide them with the knowledge that they would want in their bid to ensure that the elderly persons engage in exercises while reducing risks of falls. That serves as a clear justification focusing on why it would be important to engage in this project, which seeks to focus much attention on community education. The project will not only seek to engage the elderly population but will also involve the other populations with the sole focus being towards providing them with the knowledge that they would utilize to improving the quality of life.

Description of Where and When 

The project will be implemented in Bakersfield, California. According to census data associated with this city, the elderly population is increasing at a rapid rate when compared to any other age category. The implementation of this project in Bakersfield means that it will have more benefit towards ensuring that the rising elderly population is well protected from the risks and complications that it faces moving into the future. As has been pointed out earlier, the project is estimated to take a maximum period of 30 days. The project will be implemented between 1 st and 30 th September 2019, during which time members of the Bakersfield community will go through training on exercise and fall prevention in the elderly.

Description of Teaching Materials 

Taking into account that the project will involve teaching, it is essential to evaluate some of the teaching materials that will be used as part of the community education process. Firstly, the project will make use of an overhead projector. The aim of using a projector is that it helps in ensuring that learners develop an interest in what is being taught, as the projector offers a digitalized feel of the learning process. Secondly, the teaching process will make use of flashcards that will contain vital information on what the community members will be expected to learn. During the course of the education, members of the community will be expected to go through the flashcards as a way of ensuring that they are well aware of what is being taught as part of the education project.

Nursing Actions/Interventions 

The first nursing intervention reflects on engaging in a daily exercise regimen with the intention being towards reducing the possibility of falls. Luk, Chan, & Chan (2015) take note of the fact that exercises are beneficial towards increasing muscle strength, improving awareness to the surrounding environment, and ensuring that one has more energy for balance. Consequently, this highlights the value that ought to be placed on daily exercise as one of the ways through which to improve on the capacity for the elderly persons to minimize their risks of falls.

The second nursing intervention would revolve around addressing any medical conditions that serve as risk factors for older persons to experience falls. Elderly persons must go for regular medical checkups as one of the ways through which to ensure that they are protected from the risks of falls (Donath, van Dieën, & Faude, 2016). The checkups help in the early detection of specific medical conditions that are likely to result in falls. Ultimately, this would be seen as one of the critical approaches through which to create a safe environment for the elderly persons when seeking to minimize the risks of falls.

Potential Public and Private Partnerships 

Public and private entities may seek to work together with the sole intention being towards ensuring that they promote education within the community on matters associated with exercise and falls prevention among the elderly. The partnerships may help in reaching a more significant community base considering that the capacity for education will be increased. The ultimate impact that will be achieved as a result of the partnerships is that the number of elderly persons engaging in exercises will increase significantly, which will, in turn, help towards reducing the number of falls recorded.
