Social determinants play a critical role in the development of healthcare that is focused on quality. A community-based partnership focuses on the various factors that include social and ecological perspective, improving patient and physician relationship, and the integration of population health facets into the care of the chronically ill patient. Above it, it also enables the engagement of communities using certain principles and procedures of community involvement. One area that the community-based partnership has extremely succeeded is on the promotion of health. An innovative partnership between medical and the communities have been formed to deal with a number of chronic illnesses that have resulted in death in the US. Some of these diseases include cancer, hypertension, stroke, and diabetes among others (Community-Based Partnership for Improving Chronic Disease Management). 

The collaborative approach that involves both the medical team and the community can also influence the formulation of public policies. The Community-based Care Transition Program (CCTP) is a policy that is part of the Affordable Care Act that was formed for a variety of reasons that include improving care transitions, reducing the rates of readmission, and also preventing other medical beneficiaries that are high risk (Community-based Care Transition Program). The community-based approach can lead to the formulation of policies which not only influence the efficiencies of the hospitals but also those that improve quality of healthcare.

Healthcare workers such as nurses can utilize care coordination research in the delivery of coordinated care that guarantees quality and patient-centered outcomes in the community setting. The care coordination involves several participants that include the patients, physicians, nurses, social workers, pharmacist, amongst other professionals in the provision of this kind of health. The research will ensure that the nurses appreciate the input of other healthcare workers in the setting and also increase the likelihood of collaboration (Patient Safety and Quality: An Evidence-Based Handbook for Nurses). They will also acquire adequate knowledge concerning the resources available and the roles of other participants. Finally, it also ensures that effective healthcare is provided an appropriate time, order, and setting.
