Lifestyle is a key factor in promoting good health. According to researchers and the World Health Organizations, 60% of the different factors that affect good health in individuals are related to the kind of lifestyle that they live (Jaleel et al., 2020). In the targeted community, numerous families are living unhealthy lifestyles which has resulted in the increase in cases of metabolic diseases, skeletal complications, obesity, hypertension, cardio-vascular problems and many other health related complications. Therefore, it is important to consider this close and crucial relationship between one’s lifestyle and the impact it has on their health and develop a plan that will help solve the problem while promoting health. 

The type of unhealthy lifestyle choices that are affecting most of the people in the community include unhealthy diet, smoking, excessive alcohol consumption, malnutrition and most of all, drug abuse especially by the youths. There is also a new challenge that has recently been discovered which is the misuse of technology and has resulted to a threat on both the mental and the physical health of the people in the community. It is clear that unhealthy lifestyle is an issue that needs to be handled with utmost urgency as it influences both the physical and the mental health of individuals.

Some of the ways in which these unhealthy lifestyles impact individual and community health negatively are: unhealthy diet which leads to obesity, and nutrition problems such as diabetes (Jaleel et al., 2020). The main cause of this problem is the excessive intake of fast foods and lack of incorporating a balanced diet in the daily intake of meals. Lack of body exercise is also another factor that is affecting most of the people in the community. Exercise is a treatment for most of the health problems along with a good diet (Jaleel et al., 2020). Therefore, it is important to live an active lifestyle. 

Substance abuse which includes drug abuse, excessive consumption of alcohol and smoking is another factor which may result in health problems like cardiovascular diseases, brain injury, respiratory diseases and terminal diseases like cancer ( Nyberg et al., 2020) . The misuse of technology can impact the health of an individual physically where for example continuous use of electronic devices like laptops and phones may lead to eyesight problems and result in back problems. Studies have also shown that technology is extremely addictive and hence alters the sleeping pattern of individuals and it is a known fact that enough sleep promotes good health (Sharma et al., 2017). 

Health Promotion Plan 

The plan will involve steps that will contribute to implementing lifestyle changes across the community. Step one will include creating data insights through the use of projective analytics that will simulate a virtual population displaying how certain unhealthy life choices had an impact on their health. The aim here will be to influence the members of the community to change their ways to avoid the consequences that they will have seen. Step two will involve the creation of safe walking trails and parks that individuals in the community can have easy access to hence giving them a platform for exercising which in turn promotes an active lifestyle. 

Step three will be the setting up of stalls that offer free screening services and also giving of health prevention programs in the areas where most of the members of the community pay frequent visits to hence influencing the people to check aspects on their health such as glucose levels in the case of diabetes and also their blood pressure. Step four will be developing partnership through collaboration and team work with the organizations, including local governments and institutions, that may take interest in the quest so as to create awareness among the members of the community. The final step will be to measure the success of the plan through reevaluating the data identified in step one to help identify positive changes that came about in individual lifestyle choices. 

Importance of Collaboration

Collaboration and team work is crucial in this health promotion and can be defined as a partnership formed by individuals or by organizations with an aim to create awareness and also to influence the people in the community over the factors that affect their health which in this case is their lifestyle (Liao et al., 2020). It is important because it will help serve a larger number of people within a short period of time. It will also be a very effective method in that it will help prevent health errors because there will be a number of health experts that will take part in the action. It will also improve the individual experience since there will be many experts who will have a one-on-one interaction with each person thus promoting a better outcome. It will also save on cost due to the shared workflow among the different teams. 

The different techniques of collaboration that will be incorporated are through networking and coalitions where members of different organizations will come to an agreement on ways that they can help to influence the members of the community and also create awareness on the issue at hand while still retaining their individual autonomy (Liao et al., 2020). Another technique is strategic alliance where there will be partnerships with the various organizations that share the same goal of promoting a healthy lifestyle but also ensure that they still preserve their independence (Liao et al., 2020). It is important to nurture the right type of partnership by focusing on paying attention to good communication and also building healthy relationships with the partners. 

Important Team Members 

The team members who will be included in supporting this plan are government bodies and also the existing non-governmental organizations. There will also be private organizations and companies, health promotion advocates and people from the leadership team who include politicians and upcoming ones. The health promotion plan will be rolled out by first carrying out an assessment with the aim of obtaining all the relevant information concerning the health problem in the community. It will be achieved through conducting a survey to discover the depth of the issue and how much unhealthy lifestyle has affected the health of the people in the community. 

The next step will be achieving the support from different organizations who will be interested in collaborating and establishing a committee that will play the role of managing the activity and making of important decisions. It will also be important to set the goals that are needed to achieved and also the main objectives of the process. The other step will be to create a budget and also discover the different sources of funding for the project. After this there will be a plan that will incorporate the program for the promotion of the healthy lifestyle and after that, when all factors that will determine the success of the project have been taken care of then the plan will commence. 

The benefits of an interdisciplinary approach to health promotion are that it enhances critical thinking skills in individuals and helps to come up with deeper perspectives of issues since different people are sharing their own views of a situation (Peek et al., 2020). It also helps in coming up with creative solutions to any issues that may occur and plays a big role in developing good communication skills. This in return helps to develop the best and most effective approach to ensure that the project is a success. 

Summary 

It is important for every member of the community to live a healthy lifestyle to help prevent long term illnesses and chronic diseases. The health promotion plan to be implemented will include gathering of lifestyle-related data across the community, identification of safe spaces for exercising, stalls for free screening around the community, creating collaborations that promote awareness and measuring the success of this plan through a review of previously collected data. In the quest to promote awareness of healthy lifestyle, collaboration and team work will be incorporated because it allows for a faster spread of awareness within a short period of time due to the distribution of work among the different team members. Collaboration is also effective because it is a compilation of different expertise from the different organizations thus providing factual and adequate information that will help to convince the members of the community on the importance of living a healthy lifestyle.
