That being the case, efficient caregiver evaluation is imperative to maintaining the well-being and health of care recipient. Moreover, a policy on patient discharge planning will ensure proper care is offered and in the long run producing improved outcomes for the care recipient.

Policy: The care recipient’s needs relating to post-discharge assessment will be conducted upon admittance. Assessment will be conducted by a combined team comprising of a registered nurse, the care manager, a physician, and other healthcare providers. A plan to fulfill the care recipient’s necessities will be established. In addition, interventions to accomplish the discharge planning goals will be developed. Monitoring and revision of the plan will be done as it necessitates during the stay at the hospital.

Needs assessment factors: The following criteria will be used to assess the particular and probable discharge planning needs of the parent/family:

The patient’s, caregiver’s or family’s level of understanding the medical condition and reason why the patient is hospitalized 

The mental and emotional status of both the caregiver and the care recipient. 

The expectations of the patient/family/caregiver 

Chores the care recipient is/ isn’t able to accomplish because of his/her health condition.

The stage of care requirements, that is, acute, intermediate, or long-term

Stipulation of the necessary discharge-related patient/ caregiver education 

Suitability of the home and accessibility of transport means. 

Family/caregiver evaluation information plus their commitment to executing their roles in care provision at home. 

The existence of medical equipment, availability of supplies, and other prescriptions details as specified
