What data can you gather based on available client information? 

The information provided by the client presents me with the best possible opportunity to collect a variety of data. In particular, I have to gather data regarding her ethnic or racial background, socioeconomic status (SES), symptoms of malnutrition, coronary artery disease (CAD), and peripheral arty disease (PAD), and signs of pulmonary complications and mental illness. My decision to collect data on the aforementioned diseases remains prompted by the fact that excessive smoking serves as the primary risk factor the conditions (Clare et al., 2014). Concisely, the data gathered comprises the patient’s sociopolitical, physical, and economic aspects of life. 

What questions should you ask yourself while interviewing this client? 

One of the major questions to ask during the interview includes what socioeconomic factors led the client to average one meal every day and engage in excessive cigarette smoking. Second, is the client addicted to smoking since nicotine and dependence on cocaine, opiates, and other illicit drugs are addictive (Black, 2010)? Third, is the client willing to quit smoking or make an attempt to do so? Fourth, what clinical interventions can effectively ensure the client’s physical, social, and economic well-being?

During planning, how can you, as the nurse, best assist this client? 

Nursing care plans play a central role in providing the much-needed direction when it comes to individualized approach to healthcare. Therefore, during planning, as the nurse, I can integrate a wide range of strategies into assisting the client ( Cárdenas-Valladolid et al., 2012 ). One of the methods includes helping the client identify her specific needs (Johnson, Edward, & Giandinoto, 2018). Another strategy involves helping her recognize the various barriers to quitting, such as withdrawal and social situations. 

What are the potential strategies that would assist the client to a better life? 

During the interview, the client acknowledged and appreciated the fact that her life is not only a mess but also she lacks the ability to improve it. In response, I would adopt, integrate, and implement three major strategies with the sole purpose of bettering her socioeconomic and physical health. The first strategy involves pharmacology, which would help with alleviating her weakness and other diagnoses. In essence, the intervention in question would address any of the diseases and conditions associated with smoking malnutrition. The second remedy would involve counseling to help her understand the dangers of hardcore smoking and the significant role played by balanced diet in her immune system (Clare et al., 2014). In this context, I would allow her to understand the potential and existing primary, as well as secondary effects of addiction. The counseling process would also address any of her financial or economic concerns. The third strategy would constitute a comprehensive follow-up plan to prevent relapse and ensure smoking cessation.
