Identify a community health assessment model 

The community health assessment is the practice of health assessment on a state, local, tribal, or territorial, which identifies the critical health needs and problems via a comprehensive and systematic collection, and analysis of data. An example of a community health assessment model according to CDC is the State Health Improvement Planning (SHIP) Guidance and Resources (Centers for Disease Control and Prevention, 2015). SHIP is a framework for health improvement planning at the state level. 

What are some benefits of using a specific model to assess a community? 

A specific community health assessment model such as SHIP is beneficial primarily to the members of the society. To begin with, the model improves healthcare planning at the local level so that more community members can access medical care (Sittig & Singh, 2015). The other benefit of the use of the specific model is that it emphasizes on the engagement of the community in the collaboration for planning at the system level and the identification of the healthcare assets and the individual healthcare needs of the members of the society.

What agencies should be involved in the assessment? 

One of the agencies that are involved in the community health assessment is the community health board that is responsible for the collection, analysis and the use of data to prioritize on the issues that present themselves as healthcare needs in the society (Stanhope & Lancaster, 2015). The other agencies to be involved in the assessment are the nursing agencies across the various states in the United States of America. They are concerned with the direct evaluation of the community healthcare needs. 

What is a reasonable time frame in which to complete a community-wide assessment? 

CDC holds that a complete community health assessment should take between three and five years (Centers for Disease Control and Prevention, 2015). The evaluation is meant to be long-term research on public health problems and the solutions to the issues facing the community. 

What is the role of the public nurse in the implementation of the chosen model? 

The primary role of the public nurses in the SHIP is to support the population health. The nurses are involved in the direct care of the patients who are affected by the various healthcare issues in the communities. The other role of the public nurses is to deliver preventive healthcare services to the community members affected as part of the long-term solution to their healthcare needs.
