Summary of Individual Needs

The individual needs that Ms. Jones has expressed include the need to get better and improve her ailing status as well as the need to go home as soon as possible because she misses the home environment. The social worker also asserts that Ms. Jones needs to find a consistent environment that will eliminate the disruptions that are taking a toll on her mental health. Ms. Jones also makes statements that indicate she feels alone and isolated at home and, thus, requires company. She also has needs somebody to remind her of the essential and relevant things to do in life such as taking medication, taking her meals, where she packed her car as well as paying her bills among other critical elements. Failure to do such acts can worsen her healthcare and safe stay as well as break the law such as in failure for her to pay her bills and get relevant critical services to her.

The family of Ms. Jones is unsure of how her care will be funded as Ms. Jones social security is only able to cover for food each month and her rent because her Medicare benefits are covering her therapy that once they get depleted would present a challenge to the family. The family is in need of financial support to implement the placement concern. The family is also concerned and need of safety that is missing currently in her home because she is alone. They are also in need of her leaving the hospital as her stay in the hospital has also disoriented and affected them because of the uncomfortable environment that is deteriorating her health rapidly. The family also expresses the need for recognition from their mother who at times has found difficulties remembering her children as well as other concerns related to memory and cognitive failures.

Summary of Long Term Care Options

The long-term care options present in-depth issues that need to be done by the family as well as Ms. Jones among other members such as health care providers and the eldercare homes that she will be placed. The family has to look for means and sources funding that they can sustain her in the eldercare centers of their choice that meets the needs of their mother. Many elder centers offer various services and having different facilities and, therefore, the family have to choose one that would make them comfortable, and sustainable able to meet the needs of their mother related to cognitive status, social needs, and care needs that the family can afford and sustain with ease. For example, her present mental status of mild dementia would make her get up at night and leave alone that might increase her vulnerabilities making her more unsafe. The home, thus, needs to the one that has an alarm among other measures that can signal movements or anything relevant that enhances safety and security.

The elder care as well needs to have reminders and persons that ensure that the people they serve comply with the heath care requirements such as the taking of medication at the correct time and the right medication as well as correct quantity as stated by physicians. The family, therefore, needs to find an elder care that has qualified care provider such as a nurse, clinician or doctor that would closely monitor their clients and ensure that they subscribe and follow the care requirements that they need such as noted in Ms. Jones case. Since Ms. Jones is still recovering from her therapy and treatment from her recent surgery that repaired her broken ankle, the eldercare homes with clinicians or nurses, thus, would help her cope and take care of herself. They offer the professional point of view that can also monitor her healthcare needs and present status and, thus, take drastic action or advise the family on the possible action that they can take. It would, hence, serve the interest of the family as well as the Ms. Jones enabling them the address and handled the issue objectively. The elder care also if they would allow pets and allow her to carry her cat would help her socialize and get entertained and busy. Eldercare, thus that allows people to bring their pets along with them would hence be encouraged and chosen by the family.

Recommendation for Placement Based on Individuals Needs Taking Social, Legal, and Financial/Insurance Status into Account

  • The family should provide an estimated budget and finance that they can afford so that it determines the elder care that they chose to take their mother Ms. Jones that meets or lies within their budget. Ms. Jones should also transfer and direct her social security to help the family cover for the costs because it provides and caters for rent as well as food. The costs should be transferred to help settle the financial needs in the elder care home.

  • The elder care that cater for her social statuses such as those that allows for clients to come with pets or that provide the pets that the clients can use to help them get busy and eliminate boredom that can be experienced as result of loneliness.

  • The elder care that Ms. Jones would take, hence, should be able to provide proof and legal support in the documentation that ascertains the services that they provide so that they become sure with the services that they can get from such services and facility to help them address their need. It would assist them to address any legal challenges that might result in the future, and they need to keep a copy of the documentation and business agreements.
