Based on your performance and the expert feedback in your HISTORY collection, describe two missed questions and your understanding of why they were important to collect for this case history. Use specific references from your text to explain.
During my History collection, I missed asking the patient whether there was any significant change in her weight because insomnia, fatigue, and loss of her husband had a high probability of a depression case and depression tends to interfere with a person’s body weight. This question would have been important because it would have helped in ruling out the case of depression. I also missed asking the patient about any history of chronic diseases like diabetes as a significant number of patients with chronic diseases tend to be at a high risk of depression.

Discussion Question 2
Based on your performance and the expert feedback in your PHYSICALEXAM collection, describe two errors in your exam performance or documentation. Use specific references from your text to explain the importance of these findings in the correct assessment of this client.
Because I missed asking about the weight change of the patient, my documentation and record will miss such vital information that might lead to misdiagnoses of the patient illness. A thorough examination of patient symptoms lowers the probability of misdiagnoses (Nasrallah, 2015).Additionally, my physical exam contains an error of omission about any form of chronic disease, the patient may receive medications that may contradict and react with the medicines the patient is already using in the management of the chronic illness in case a chronic illness exists.
Discussion Question 3
Based on your performance on the PHYSICAL EXAM collection, describe one key finding that you included in your list, and describe a specific physical exam that you can perform at the point-of-care to further evaluate the finding. Use specific references from your text.
In my physical examination I found out that the patient exhibited symptoms that were related to depression, however, I also noted that such symptoms are highly related to hypothyroidism, therefore, checking the level of the thyroid-stimulating hormone will help in evaluating my findings and giving a correct diagnosis of the patient condition.
Discussion Question 4
Based on your performance and the expert feedback in your ASSESSMENT identification of problem categories, choose one missed/incorrect category and use specific references from your text to explain the importance of this category in arriving at correct differential diagnoses for this client.
In my history collection, I failed to ask the patient about the significant changes in her weight. This was important, as it would have helped in determining whether the patient was suffering from depression or other diseases that exhibit similar symptoms as depression for instance hypothyroidism.
Discussion Question 5
Based on your performance and the expert feedback in your ASSESSMENT of differential diagnoses, describe one incorrect/missed differential diagnosis and use specific references from your text to support the inclusion of the diagnosis for this client.
In my assessment, I missed the differential diagnoses of Parkinson disease. The patient exhibited depressive symptoms but not all depressive symptoms are related to depression as a patient in the early stages of Parkinson disease tends to exhibit mild to moderate depressive symptoms. It is essential to scrutinize the patient’s symptoms and connect them to any possibility of diagnosis (Acharya et al., 2015). This ensures a proper diagnosis of the patient's condition.
