The Vaccine Information statement (VIS) of Tetanus, Diphtheria, and Pertussis is a very important factor in the eradication of these diseases. Case Study: Tetanus
John, a 45-year old man, under the abuse of injected drugs, and living alone following divorce with his wife Mary 10 years ago for alleged abuse of drugs. The patient was admitted to St. Teresa Hospital in emergency ward following complaints of pain and stiffness on face and back muscles the past three days. During the entrance time, John was conscious with the medical history showing no record of vaccinations and taking of drugs. The vital signs exhibited by the patients were stable. The temperature taken orally was 37.5ᴼC. In physical examination, John had a wound with diameter of 3×4cm within his right inguinal with secretion of pus. Observation on the jaw muscles showed presence of spasms and he was not in a position of opening his mouth. There were intermittent spasms within lumbar muscles which were beginning through any stimulation and slowly added to seriousness of the contractions and the periods between contractions became minimal.
Following the lab test, the laboratory data provided were as follows:

CRP (C - reactive protein) = 9mg/dl
Urea= 34mg/dl
Cr (Creatinine) = 0.8 mg/dl
Ca (Calcium) = 8.9 mg/dl
Na (Sodium) = 139mEq/L
HIV Ab: Negative
HCV Ab: Negative
HBs Ag: Negative
The patient was hospitalized in an ICU with tetanus diagnosis. He was prescribed for diazepam and Midazolam for spasms controlling. Furthermore, Metronidazole 500mg was provided intravenously after every 6 hours for the wound. Tetanus Immune Globulin (TIG) 500 IU and a single dose of Tetanus and Diptheria (Td) vaccine were also given (Loughlin and Generali, 2007). However, despite of all intubation and drug therapy, the client was expired due to autonomic system disorder and cardiac arrest.
