While transitioning in healthcare is often necessitated by the need to improve care provision, ensure patient-centered care, and influence efficient utilization of resources, the projected results are unlikely to be achieved unless strategic management of transitional undertakings is ensured. One of the aims of an outpatient service model is to reduce the Per Capita cost of healthcare (Baldonado et al., 2017). It is impossible to achieve this goal without collaborative care. The Transitional Care Manager (TCM) should hence be proactive in ensuring outreach assistance, solve problems among community members and educate individuals on how they can take advantage of the model. The manager should also ensure strategies are put in place to improve patient outcomes and increase care access. The healthcare system's complexity often derails patients with complex health conditions from receiving the care they deserve (Baldonado et al., 2017). The Transitional Care Manager (TCM) should ensure that the qualified physicians selected to deal with patients pursue evidence-based approaches that reduce admission rates and frequent use of emergency centers.
Acquisition & Recruiting
The need to ensure outpatient physicians are equipped to handle healthcare complexities and be resilient and flexible in the ever-changing healthcare environment is critical. Acquisition and recruiting should hence be a deliberate, well thought out process that is strategized around. To ensure talent and professionalism are achieved in the recruitment process, there is a need to follow the employee recruitment process model. The human resource management team should first set the recruitment process's objectives, a recruitment strategy developed, recruiting carried and measurement of recruitment results (Breaugh et al., 2014) . In following this process, besides ensuring professionalism and passion in care provision, the recruiting team should also consider employees conversant with telehealth. Successful outpatient care provision can be complemented by telehealth. Technologically savvy individuals, hence, should be given a priority. Regardless of the above considerations to ensure value addition to patients, well-rounded employees should be sought.
