I will begin with requesting the family members to help me with family information for designing a genogram. The goal will be to understand how the entire family relates to each other. This will help me understand who lives with whom and provide a psychological insight to the factors that reveal members in the family with similar issues. It will also be easy to know the active family members in the life of the patients. The genogram is critical as it reveals family members who are in the life of the patient but are not currently at home. For this case, the genogram will have the great grandma at 95 years old, grandma at 75 years old, grandpa at 80 years, brother 55 years, mom at 53 years and son at 20 years old.

Who would you define to be the primary client? The secondary client? How do their needs differ?
The primary client is the grandma while the secondary client is the great grandma. Their needs differ in that the great grandma entirely depends on the grandma for everything as she is not able to run anything in her life due to her age. Grandma is capable of providing for the grandpa and great grandma who live with her in the same house.
Who would you involve in assessment and care planning? What would be the primary areas that you would first assess?
In assessment care planning, I will involve the family members for primary questions regarding the family structure and function. The primary area to assess would be the family structure, cultural history of the family and medical history of the family.

How would you approach the different issues faced by family members across four generations and different life stages?
With the help of the genogram, I will be able to identify the source of the existing issues and advice on the way forward regarding treatment. This will be easy through interviewing family members to understand the presenting problems and underlying issues and how they connect to each other in the family.
What would you do to ensure that your assessment and interventions are culturally competent?
Multicultural factors are key when obtaining the patient's clinical history. My assessment and interventions should be culturally competent to allow easy planning of a treatment intervention. I will find out levels of acculturation, blended cultures, and generational status within the family. This will be through asking direct questions to the family members. I will find out caregiving norms in relation to the culture as I understand that the norms vary depending on the culture. Additionally, I will find out from family members about their view regarding cultural expectations on caregiving.
