Subject: Fire Accident Report
On the 13 th December 2020, at around 1400hours in the afternoon, a fire broke in the East wing following a big sound of a burst originating from the wind. When the fire occurred, I was in the storeroom counterchecking the remaining stock from the last stocking and assessing whether the safety measures were maintained in the recent stocking storehouse. I immediately received a call from the head of the east wing departments and rushed to the place to see what transpired. It took me less than ten minutes to arrive at the wing, but the fire had spread to the basement by the time I got there. We quickly contacted emergency help to put the fire off, and the support was available within a short time. The fire was put out within about ten minutes. Unfortunately, two of the staff members working in the basement sustained minor superficial burns and were rushed to the hospital for urgent treatment, where they are receiving care even now.
I undertook to investigate the fire's cause, teaming up with the East Wing manager together with the emergency team and the quality control team. Through the investigation, we realized that the ammonium nitrate tanks in stall seventeen next to the basement had been poorly stored; hence they caused the fire. This prompted us to check the East Wing's storage reports, which revealed that the ammonium nitrate tanks were poorly placed and the chemical inside them was not up to the recommended level. Two of the tanks were excessively filled, while another tank's seal was not compatible with the tanks. The condition, however, had been reported though no serious actions had been considered yet. This negligence of duty has caused the company significant damage.
