Demographic Information 

Karen is 26-year-old Caucasian, single mother of 3. She currently lives with her children in Los Angeles in a middle-class neighborhood. She is a former substance abuser of meth and has had two encounters with the police and DFCS that led to her detainment. She had also lost her children during her encounter with DFCS, but later got them. She is presently rehabilitating at SAFFT and has an ongoing involvement with SAFFT and DFCS.

Referral Source and Presenting Concern 

Karen was referred to SAFFT by her former cellmate in prison who told her she used the agency to talk to her children before she got arrested. Once free, Karen contacted SAFFT and admitted herself in all of SAFFT’s rehabilitation programs. Karen came to SAFFT seeking an avenue for change in her life. Being an ex-convict and a substance abuser, she hoped SAFFT would help her organize her life and even get to talk to her children. 

Description of Presenting Concern 

Karen is a recovering meth addict who has been convicted twice for dealing. She started substance abuse when she gave birth to her first child as a coping strategy to the hard life she was experiencing. Karen came to SAFFT in an aim to get her life together and get to see her children. The loss of her children hit her really hard and served as the wake-up call to change her life for the better. While in jail, Karen used to attend AA meetings and church services. Currently, Karen leans on SAFFT for help in making tough decisions concerning her life. 

Social History 

Karen was a brilliant student at her school and was among the top performers. Karen has been involved in an unstable family of alcoholics. She got pregnant at 15 and almost dropped out of school. She has been in an abusive relationship where the boyfriend cheated on her, and they separated, leaving her with three children to take care of. She turned to drugs and got addicted. 

She has had two encounters with DFCS, and the recent meeting led to her children being taken away into foster care. She has also done time twice as a result of possession of drugs. 

Observational Assessment 

Karen is quite alert and very conscious of her actions and words. She can explain herself and her situation with a little incentive clearly. Her past life gives her pain, and she even cries when telling her story. However, a few positive incidences in her life make her feel lively. 

Formulation 

Based on the client’s report, the client is regretful for the poor decisions she made in her past life. Karen is, however, a strong and serious woman who managed to use the opportunity given by SAFFT to get her life together. She calls and visits often and feels indebted to SAFFT for the positive change it has brought to her life. She is, however, a recovering addict who needs to avoid all triggers that may lead to relapse. 

Treatment Plan 

Karen will continue being enrolled in SAFFT programs for six months, and her progress will be monitored during this duration. Since Karen is cooperative and loves SAFFT agents, the secretary at SAFFT will serve as her focal point because they have associated severally during her time at SAFFT and are now friends. 

Karen’s family will also be contacted to ensure she stays away from any triggers that may lead to her relapse during the course of her rehabilitation. A counselor visit for nine consecutive weeks is also recommended to help her overcome the guilt and regret she feels. 

Conclusion 

Karen is a 26-year old Caucasian single mother of 3 children. She was brought up in an unstable family of alcoholics and her parents often fought. She got pregnant at 15 and almost dropped out of school. She has been involved in an abusive relationship. She got introduced to meth by her friend, and this led to addiction. Twice she has been involved with DFCS, and her children got taken away, and she put in prison. It was in prison when she heard about SAFFT and voluntarily enrolled in SAFFT programs once she was released. Her cooperation has led to positive change, and she even got her children back two months ago. Karen is a strong, smart woman with responsibility for her children. She hopes to get better and believes that SAFFT is the key.
